Comprised of experienced nurses and social workers, the team assists with care coordination, community resources, transportation needs, specialist follow-up, benefit assessments, long-term care planning, hospice referrals, and much more. By maintaining regular contact with patients between office visits, the Care Management Team helps ensure that important needs are identified and addressed before they become larger obstacles.
